As your medical home, we deliver care that meets the five requirements established by the U.S. Agency for Healthcare Research and Quality. We provide care that is:
Patient-centered
We are your partners in care. We work with you to manage your health and make sure decisions about your care reflect your needs, wants, and preferences. We also provide education and support so you can be an active participant in your healthcare.We take a team-based approach so we can meet most of your healthcare needs throughout your lifetime. Our team members provide prevention and wellness services, acute and chronic care, rehabilitation, and behavioral health care.
Coordinated
We coordinate your care with providers and organizations across the healthcare system, from specialists to hospitals and rehabilitation centers to home healthcare and community support service providers. Together, we help you avoid duplicate tests. Even more important, we work together to make sure you receive the right care in the right place at the right time.
Accessible
We make it as easy as possible to get care and information when you need them. We deliver urgent care with shorter waiting times and we have extended hours for certain services. We also offer 24/7 email or telephone access.
High Quality
Our providers and staff share an overriding commitment to quality and safety. We practice evidence-based medicine, care deeply about patient outcomes and satisfaction, and constantly seek new ways to enhance your patient experience.

Join our Patient Advisory Council and help enhance quality
As part of our commitment to quality improvement, we continually assess our performance, identify areas for improvement, make changes—and assess again. Because we’re a patient-centered medical home, we involve patients in these quality monitoring and improvement efforts as members of our Patient Advisory Council. It literally gives patients a seat at the table and an opportunity to impact the planning, delivery, and evaluation of patient care.
